Spring Registration is open closed

Bring completed REGISTRATION FORM, payment, copy of Birth Certificate & Medical Waiver form to the office.

Age groups may be combined based on number of players in age groups.

U5: DOB between 8/1/07 – 7/31/08 U6: DOB between 8/1/06 – 7/31/07

U7: DOB between 8/1/05 – 7/31/06                U8: DOB between 8/1/04 – 7/31/05

U9: DOB between 8/1/03 -7/31/04                  U10: DOB between 8/1/02 - 7/31/03

U11: DOB between 8/1/01 - 7/31/02               U12: DOB between 8/1/00 - 7/31/01

FEES:

FALL 2012:                                  $75 per player (includes Uniform, Award)
FALL 2012 & SPRING 2013:     $110 per player
SPRING 2013:                            $75 per player

* $5 sibling discount available to families with multiple registrants

1. Print & complete REGISTRATION FORM

2. Print & complete the Medical Waiver Form

3. Make a copy of child’s birth certificate

4. Write a check ($75) to G.U.S.C and deliver or mail to:
