1. Print the FALL REGISTRATION form and mail to the the Club office. (You can also email completed form to GUSCrecreationsoccer@gmail.com.)
2. Complete the MEDICAL WAIVER form
3. Print and sign the "I have read the Parent Handbook" form
Don't forget to bring:
-- Copy of birth certificate (include in registration packet)
-- read code of conduct
All volunteer coaches must complete a risk management form and make a copy of their drivers license to submit to the GUSC office. The form can be completed by going to www.ncsoccer.org
